Getting started

The intake form covers medical history, medications, skin conditions, allergies, and your goals for the appointment. It takes about 5 minutes to complete.

After submitting, we'll review it and confirm your appointment within 24 hours. If you've already booked a session, completing this ahead of time means you'll have extra time with Soza for design and consultation.
